Mission Treasury Service Remittance Form (for Basic Mission Pledge, Christmas Joy, One Great Hour, Pentecost, Peacemaking, etc.- do not use for sending Per Capita as that goes directly to Presbytery) PDF format *Send with every remittance, be sure it's filled out correctly so that funds are directed as you wish
